I grant permission:
● I give my permission for this volunteer to participate in Vacation Bible School taking place July 21th to July 25th 2023. I hereby waive and release, indemnify, hold harmless and forever discharge East Bergen Christian Church and its agents, officers, directors, employees, volunteers, affiliates, of and from all claims, demands, debts, contracts, expenses, causes of action, damages and liabilities of every kind and nature.
● In the event of illness or accident, having parental or self responsibility for the below named volunteer, I give permission for first aid to be administered where considered necessary by a person trained in first aid, if available, or medical treatment to be administered by a suitably qualified medical practitioner.
● If I cannot be contacted and this volunteer should require emergency hospital treatment, I authorize an adult leader to sign on my behalf any written form of consent required by the hospital. However, I understand that every effort will be made to contact me as soon as possible.
● I understand that by myself or my child volunteering at VBS photos & short video clips are taken of myself or the children in various activities. I am accepting that these photos may be shared on EBCC's website, Facebook page, or used in other social media or church publications. No names of volunteer will be given.
